Purpose:
HDx Labs (“we,” “our,” “us”) is committed to delivering high-quality services at affordable prices, ensuring increased patient accessibility. The purpose of these policies is to establish clear guidelines for billing and account collections. We are dedicated to upholding compliance, patient satisfaction, and operational efficiency. Our primary objective is to diligently, reasonably, and fairly inform patients about their financial responsibilities and available assistance options. We also pledge to make reasonable efforts to assess a patient’s eligibility for financial assistance through our Financial Hardship Program before initiating collection efforts.
Policy:
Upon the provision of services, our policy is to bill relevant third-party payers as required by our contractual and/or legal obligations, or to directly bill patients when applicable, in a timely manner. Patients may be responsible for deductibles, co-insurance, or co-pays after insurance processing. In cases of delinquent or unpaid accounts, or outstanding patient obligations, we will make three (3) additional collection attempts per date of service, sending monthly statements. At present, we do not anticipate employing a collection agency, but we retain the right to consider it in the future.
Billing Practices:
A. Insurance Billing:
For all insured patients, HDx Labs will bill appropriate third-party payers in compliance with our contractual and/or legal obligations and based on the patient-provided or verified information. Claims will be processed according to the policies and procedures of each specific third-party payer. We will only submit a claim to the patient if directed to do so or if permitted or required under current regulations and industry standards.
B. Self-Pay Patient Accounts:
Self-pay patient accounts include those without insurance coverage, patients covered by third-party payors with which we do not participate, or patients designating themselves as self-pay. All self-pay patient samples will be processed upon receipt by HDx Labs or the referral laboratory. Self-pay patients will be billed based on the HDx Labs chargemaster, following the policy outlined below. Self-pay accounts have the option to benefit from a prompt-pay discount (if paid within ninety (90) days from the billing date), as per the listed fees below.

C. Financial Hardship Program:
For patients with co-pays, co-insurance, deductibles, or other financial obligations, whether before or after insurance processing, timely payment is required. However, recognizing patient financial constraints, we offer a Financial Hardship Program (the “Program”) as detailed below. This Program is provided free of charge.
To participate in the Financial Hardship Program, patients must choose one of the following options:

2. Call 904.588.2164 to speak with the HDx Labs Client Account Manager
3. Email contact@hdxnow.com to request a financial hardship application form that can be completed by the patient and returned via email.
4. Mail the completed form with supporting documentation to HDx Labs Executive Office, 8465 Merchant’s Way, Suite 206 Jacksonville, FL 32222.
For any questions regarding the application, patients can contact HDX Labs Client Account Manager by calling 904.588.2164 (option 4). HDx Labs will determine patient program eligibility at its sole discretion. Patients may be eligible for up to a seventy-five percent (75%) reduction of their bill based on their Total Adjusted Gross Household Income.
We retain the right to request proof of income, including but not limited to a copy of the patient’s W2, three (3) months of pay stubs, or the patient’s most recent tax return, for hardship status approval and discount eligibility based on the sliding scale provided above. The sliding scale is subject to change at our sole discretion.
